Why did Uchiha Madara appear young after his resurrection?
It was because the Rinne Tensei no Jutsu had been specially prepared for him.
When resurrected using this technique, Nagato's chakra and life force were extracted and infused into Madara's body.
It was similar to how Nagato had absorbed Bee's chakra after the original Edo Tensei.
After taking in such a massive amount of chakra rich in vitality, it wasn't difficult for Madara's body to revert to a younger state.
However, prior to this, Madara had given his last Sharingan to Obito and was originally meant to be revived using Edo Tensei. But under Orochimaru's control, he was directly resurrected instead.
So even though he had now fully returned to life, his body had also reverted to its younger form.
But it was a body without eyes.

"I see." Madara frowned. "It looks like I'll have to stretch my muscles a bit before my Rinnegan is returned."
Uchiha Madara was incredibly powerful.
But at the same time, he was also pitiful.
Because no matter how strong he was, in the end, he was nothing more than a pawn of Black Zetsu.

"I fought against him in my youth, I could never mistake him," Ōnoki said, cold sweat forming on his forehead as he subconsciously swallowed.
Ōnoki had never forgotten the fear Madara instilled in him.
After all, in his younger days, he had been utterly terrified of Madara.
But why was Uchiha Madara here?
It didn't make sense!
At the foot of the mountain, feeling the raw physical sensation coursing through his body, Uchiha Madara clenched his fists in excitement.
"It's been too long since I've felt this. Are these the same Five Kage from before? Hmph, I suppose I can entertain myself with them."
"Kakashi, step aside."
Minato gave the decisive command.

A few kilometers away, hidden behind a massive boulder, Orochimaru, Nagato, Tsunade, Mikoto, and Black Zetsu were gathered.
Orochimaru sat cross-legged on the ground, pressing his hands against Nagato's back, using Yin-Yang Release to restore his depleted vitality.
"As expected, my hypothesis was correct. This technique has a direct contractual connection with the Pure Land. Using it is even more draining than extracting a tailed beast."
"Can he be saved?"
"It's possible, but it will take time." Orochimaru said seriously. "Here's the plan, Tsunade, Mikoto, you two head over and provide support. If Minato and the others can't defeat him, step in."
"Got it. Mikoto, let's go!"
Tsunade nodded decisively and rushed toward Madara with Mikoto.
Watching them leave, Orochimaru continued, "Zetsu, to avoid raising Madara's suspicions, deliver Nagato's Rinnegan to him now."
"Send it now?" Black Zetsu hesitated, glancing in the direction Tsunade had gone. "Then what about the safety of your wives…?"
"It'll be fine. The Rinnegan is powerful, but Madara has just been revived. It will take time for him to adjust to it. Tsunade and the others can handle it."
