Surgery Godfather-Chapter 1994 - 1351: Is This Really a Township Hospital? (Part 2)
"Understood, we’ve already gotten in the car, let’s talk more when we arrive."
Director Zhang Anyun is a man of understanding, not wasting words or being verbose. They’re going anyway, so they’ll discuss more upon arrival. He, along with an Anesthesiologist, an Instrumental Nurse, and two Assistants, quickly got into the ambulance arranged by the Hospital, bringing various types of blood products, mostly Type O, and hurriedly set off.
...
Li Min stood by the Operating Table, quickly opened the Abdominal Cavity, blood gushed out in a blur.
Suction and compression were simultaneously applied, and in the brief clear view, he discovered a great diagonal tear on the Spleen still oozing blood; there were three lacerations on the outer edge of the Left Hepatic Lobe, fortunately not too deep. He took a deep breath.
This type of Surgery was what he was most skilled at, having performed numerous times at Sanbo Hospital.
"Free the splenic hilum, sever the splenic Artery first."
He planned to proceed with Spleen Resection.
The Elderly Director followed with Vascular Clamps, forty years of experience and still competent as an Assistant.
Separate, ligate, cut. At the moment the splenic Artery pulsation stopped, the Spleen deflated like a punctured ball, and bleeding diminished significantly. Completely removed, taken out, the Abdominal Cavity was temporarily clean.
But Blood Pressure was still dropping. 𝓯𝓻𝒆𝙚𝒘𝓮𝙗𝓷𝒐𝓿𝙚𝒍.𝙘𝓸𝙢
Li Min’s gaze swept towards the Liver. Those lacerations were visibly expanding, and the blood surrounding the Liver rapidly pooled again.
"Liver repair, 4-0 Prolene!"
The Elderly Director’s hand, holding the Needle Holder, trembled slightly.
Suturing, knotting, suturing, knotting. Li Min’s fingers guided the needle and thread over the soft Liver surface like a seamstress stitching on silk. Three lacerations, seven "8" shaped sutures, the bleeding stopped.
Flush with warm saline, inspect the wound, confirm no active bleeding, place a Drainage Tube in the splenic nest.
"Close the abdomen!" Li Min straightened his back, sweat from his forehead dripped onto the edge of his sterile cap.
The Elderly Director took the Needle Holder.
"You go," he said, "I’ll handle the rest here."
Having done forty years of Surgery, the Elderly Director was confident in handling the finishing tasks and closing the abdomen.
Li Min returned to the first Operating Room, uncovered, and began closing the chest.
Sternal alignment, wire fixation, muscle layer-by-layer suturing, subcutaneous tissue alignment, intradermal continuous suturing. Thirty minutes later, the monitor’s numbers remained stable.
The Anesthesiologist whispered, "Okay to send to the Recovery Room."
The Anesthesiologist’s back was completely soaked, never having experienced such a situation, or participated in such a Surgery, only managed to follow Li Min’s guidance bravely.
Li Min stood by the Operating Table, together they smoothly transferred Zhou Fusheng, watching his chest rise and fall evenly with the ventilator assistance. His ash-gray face had regained color, and his tightly knit brows relaxed.
After finishing these two Surgeries, immediately send them to the Ward.
The Operating Room was vacated, and Li Min was onto the next two cases, one was Xu Dehou and the other Zhao Qiulin.
The Operating Room was quickly cleaned, Xu Dehou was brought in, lying on the table, the collapsed side of the chest had been temporarily fixed, breathing was still labored. Bedside ultrasound showed considerable blood had accumulated in the chest again.
Li Min quickly reviewed the newly released CT images: Fractures in the right 3rd to 7th ribs, with noticeable dislocation at the ends of the 4th and 5th ribs, piercing the parietal pleura, with two contusion lacerations on the lung surface.
Open the chest!
No need for further discussion.
Li Min moved swiftly, opening the chest cavity, gradually suctioning away the accumulated blood, revealing two lacerations on the lung surface still slowly oozing blood, electrocautery, hemostasis, the rib ends were repositioned, wires passed through, and tightened.
The Anesthesiologist reported Vital Signs: "Stable Blood Pressure, Oxygen Saturation 99%."
He laid down the Needle Holder, watching the screen as the lung lobes re-expanded under the Anesthesiologist’s lung inflation, looking pink and full.
The adjacent case was merely Fracture, the Elderly Director could fully handle it early.
Li Min wasn’t in a hurry, completing Xu Dehou’s chest closure and placing of Drainage.
"Dr. Li, Zhao Qiulin’s side is ready, the Elderly Director has already prepared the sheets, just waiting on you."
Next door, Zhao Qiulin lay on the table, the open Fracture on the left thigh had been cleaned, the wound covered with sterile dressings. The Elderly Director stood in the first assistant’s position, the Instrumental Nurse had prepared the plate and screws.
Li Min changed into a new Surgical Gown, stood at the Chief Surgeon position.
Open femoral Fracture, Gustilo IIIA type. Such an injury isn’t the most challenging in a Provincial City’s top-tier hospital, but at a township health center, it’s a Surgery capable of causing any Surgeon to break out in a sweat.
He picked up the Bone Holder.
Reposition, temporary fixation, fluoroscopy. Plate shaping, application, drilling, measuring depth, tapping, screw insertion.
He worked quickly, yet steadily.
The new C-arm X-ray machine was used for the first time, with the Doctors below executing intra-operative fluoroscopy under Li Min’s guidance.
The last screw tightened, fluoroscopy showed satisfactory alignment and positioning of the Fracture and the plate. Flush, drainage, layer-by-layer suturing.
Li Min put down the Needle Holder.
"Observe for fifteen minutes, if there are no issues, we can send them to the Ward," he said.
He exited the Operating Room, leaned against the corridor wall.
Four Surgeries, from Emergency reception to finish, took three hours and seventeen minutes. His Surgical Gown was soaked through, adhering coolly to his back. His hands still trembled slightly, not from nervousness, but muscle memory. The feel of the needle passing through Myocardium still lingered at his fingertips.
The Elderly Director followed him out of the Operating Room, silently watching him.
"Why are you looking at me?" Li Min asked.
The Elderly Director still didn’t speak. He reached out, adjusting Li Min’s lopsided Surgical cap.
"Let’s go to the Recovery Room."
The post-surgery patients were all placed here for concentrated care, safer than directly sending to a Ward.
They arrived at the Recovery Room, the previous two severely injured patients, Zhou Fusheng and Chen Dongxiu, had stable Vital Signs now.
